Eric,
AZ DOT returned my form and check today indicating that the fee was NOT $5.37 (as was told to me over the phone) but $24.00 (uncertified history) OR $40.00 (certified history).
Is this a good sign that they may have more than 1 record in their database?
(i.e. 8 records for the car)
I'll be sending the form back in again with additional funds. I find it interesting that they play this "cat & mouse" game by sending the form and original check back to the originator asking for more money. I guess if I was a resident of Arizona it would be a easy "walk in" and they could tell me what it was going to cost on the spot. I guess they want to make sure I'm not a "kook".
