Which way is the large, convex washer oriented on the balancer, and is it an actual lock washer under the bolt or a star washer? (the attachment kit from Long Island came with the latter).

The washer of which you speak is not actually convex. It is just a crudely stamped, special, large flat washer. The way that these things are stamped out, they may APPEAR to be slightly convex but I think that's just because of the crude stamping. In any event, if I use one of these pieces, I usually put the "concave" side toward the balancer. If you get a high quality piece (like an ARP-manufactured bolt and flat washer) the washer will not appear "convex"; it will appear FLAT.
I believe that these balancer bolt assemblies DID originally use an external tooth star washer. I NEVER use such a washer, though, for this assembly. Star washers are low grade pieces and I would never use one in conjunction with this sort of application. But, there are likely millions of them out there performing alright so if you want to use it, go ahead. GM used this star washer on MANY of these balancer bolt applications for years, small block and big block. I'll NEVER understand why, though.
