I'm not sure there's any reason to blame yourself. If the pieces are still around, you might want to gather them up and save them. If it ever became relevant, you or Delta could have an expert inspect and test them in order to render an opinion as to the cause. I realize that right now you have a lot to be grateful for, and I'm not for a second suggesting litigation or anything else, but it might be worth hanging on to the 'evidence,' for whatever reason.
Even a tiny nick during manufacture/installation or shipping can create a stress riser and the possibility of failure, and undoubtedly bad batches of wheels also exist. Court cases over the reasons for similar incidents are fairly common with experts on each side reaching drastically different conclusions.
Your post served to restress the dangers of power tools, even when used with care and as intended. Glad it wasn't any worse.
