The jack in my 70 looks like one listed on ebay, item #4545435381. I know it's not correct for my car, but does anyone know what car it came with?
I'm also on the look-out for an original dated 0A to 0D if any suggestions on where to look besides ebay or Driveline.
Thanks!

The handle/wrench is a MUCH later design. In fact, I think that you can still get this one from GM under GM #14050029, but I'm not certain because I can't tell what wrench size it has.
The jack is a much later style jack. It could be one from a number of GM cars, possibly from the early-to-mid 80's, or so. It may even be an early C4 jack. The saddle-type lifting pad identifies it as designed for a lifting configuration that didn't exist in the 60s and 70s.
