He wants as many people that have had problems with failures, weld cracks, etc. in thier hitches to give him a call or E-mail him. Some of the form may not apply if you did not have a wreck as a result of failure.

May be a trend of defects reported that is being followed up by the NHTSA.
May be a swell idea to crawl under your Chevy/GMC critter with an OEM hitch and poke around. I seem to recall someone in NCRS had this issue a while back.
