Help Identify Radio Tag Card 67-69?

Hey guys,
I picked this up recently (wasn't a lot of money but was assured to be an original). I wanted one for my silver 67 unrestored car (Dec 66 build) and thought this would fit the bill for it. I knew that it differed in a couple of ways from original Corvette cards, but took a chance. A good buddy, and fellow NCRS member didn't think it is correct for a Corvette, but wanted to know if anyone on here could tell me what it actually is?

The part # called out on the 67 radio option for the antenna trim tag is 3886966 per the 67 AIM
