Only black interior was available with Daytona yellow but i wanted saddle interior, so they built a saddle interior car with Special paint i guess that's how they got around it. See the ink pen line drawn through daytona yellow & saddle interior with the initial D next to it

Yes, SPEARS is printed close to the bottom of the sheet.

I didn't look far enough down on the document; it looks like another name is farther up the body of the document. Very few documents have the owners name on them unless they were a GM or supplier ordered car. The Bart Star and Alan Bean Corvettes have their names on the paperwork for their cars.
