Can anyone identify what this shifter belongs to?

It has a reverse lockout - just like a Vette

Buick Grand Sport in the mid 60's had a reverse lock out like a Vett, may be off one of them.

My 1965 Impala had a reverse lock-out, but it is not that shifter -- wrong shape to the handle.

Looks like Joel is correct !!

I found a Vega shifter on eBay and the mounting plate/shifter handle is exactly the same.

While it has been a few years since I've messed with one, the way that shifter bolts to the body kinda reminded me of a Vega set-up...they used a Saginaw 4-speed, which would NOT, under most circumstances, stand up to a V-8 engine...I learned the hard way!
