Chevrolet Order Document? Info Needed

What type of document is this and were these included with the new car? Does anyone have one these with their car? Kind of looks like a tank sticker. Post a picture if you have one.

That's Copy #5 of the "Corvette Order", with the VIN, drivetrain numbers/codes and key number manually typed at the bottom; it went in the glove box to the dealer. It was computer line-printed in the same carbon stack with the window sticker and the Car Shipper, before the car was built, which is why the drivetrain info, VIN, and key number were added manually at the end of the line.

john, how many copies were included in this document if this was #5? I wonder if anyone has a original complete unused one floating around? Would make a great conversation piece.

I have an original for my 65 coupe. It looks like the one you posted but mine even has pieces of the carbon paper still attached to it. As the fifth owner of my car I feel pretty lucky that alot of original documentation has been handed down with it. I haven't heard of alot of these being around since I think the dealer would keep or destroy it once they received the car.
