Well, I pulled the clutch boot off, and I found the third hole with an awl. The holes in the firewall looked like pilot holes with never a screw in them. The boot looked like it never had any screws in it, and was just held in place with mastic. See the lower left ear:
No evidence of a screw being on this boot.
Here's the back:
Number on the front is 22 with a combined JR, and I'm pretty sure it is my original.
I think the coffee break theory on the assembly line is true.
